Why do police sometimes ride in the back of an ambulance with a patient??

i would've thought mentally, physically unstable patient.

Usually a mental escort for SJA. Depending on the patients demeanour, aggression or sedation will determine if police ride in the back.

Also to protect the Medic from some of the rotten eggs they cart off to hospital
